As per JSLI Constitution and Bylaws, we invite good standing members to file the attached Nomination Ballot for any desired position. A nominee has an option to send passport size color photo along with the form.

Completed nomination form must be sent by first class mail and must be received on or before Nov 19, 2021. Nominee to confirm the receipt of nomination form between Nov 22-Nov 24 by contacting election commissioner Shri Kalapi Shah  35 Boblee Lane Hicksville NY 11801 Cell # 516-286-8782


A Nominee to include half a page of cover letter stating what you will do for JSLI during the length of your term, if elected.
